During the G chord, throw some hammer-ons on the second fret of the A. During the C chord, hammer on the second fret of the G. If you can't work it from there, I'd be happy to tab it out.

like for the first line

it's not just [D] that he plays at the end, it's something like [D][E][D]

or maybe it's just an upstroke (i guess that is what you call it) on the top e
